He acts as a mirror for the audience, bringing our primal subconscious desires to the forefront, uncomfortably forcing us to question why we enjoy jacking cars, going on killing sprees and murdering by the hundreds. Instead of suppressing those urges, Trevor embodies that desire in it's totality. We steal cars, commit murder by the hundreds without concern for others, unbound by consequence or retribution for our actions. You said it as well, he manipulates and abuses the people around him, deals drugs, smuggles arms, murders hitchhikers and much more. Like I said previously, obviously what he does is reprehensible. He is his own man uninfluenced by those around him. He's liberated from any conceptions of ego. There's something perhaps inherently appealing about that, being completely fearless in ones actions, unshackled by any sense of humiliation or judgment from others. Trevor shows no anxiety or shame in such situations. In one cutscene, he pulls his pants down and reveals his genitals to others.
Throughout the game, you'll see him in compromising situations that we might see as embarrassing, like being stranded in the middle of nowhere half naked in a drug hazed stupor or vomiting in a fountain in public. To do what he wants when he wants without really giving a shit about what others think of him. Like the player, Trevor simply commits mayhem purely for pleasure and enjoyment of it. While most of his actions throughout the story are undeniably morally objectionable, he doesn't try to justify his sadism with any grander motivations or wrap them up with self interested moral reasoning. I think what makes Trevor an appealing character to people is that he is unabashed in his psychopathy.
